ABSTRACT

Objective@#To understand the prevalence and pattern of abnormal uterine bleeding (AUB) among female medical college students in Shanghai,to provide a basis for menstrual health intervention for female college students.@*Methods@#A total of 912 female medical students from a university in Shanghai were selected through cluster sampling. An anonymous questionnaire survey on menstrual history and related factors was conducted in March 2021.@*Results@#The prevalence of AUB among female medical college students was 39.9%, with cycle disordered and non cycle disordered AUB of 24.7% and 15.2%, respectively. Among female college students with AUB, 21.7% had two or three abnormal menstrual symptoms. Multivariate Logistic analysis showed that sleep quality was associated with both types of AUB, and female undergraduates with poorer and poor sleep quality were at increased risk of both cyclic disordered AUB ( OR =2.05) and non cyclic disordered AUB ( OR =2.00). Low BMI ( OR =1.66) and weight gain and loss ( OR =1.76) were associated with cycle disordered AUB. Frequent insomnia ( OR =2.45) was associated with non cyclic disordered AUB( P <0.05).@*Conclusion@#Abnormal uterine bleeding (AUB) is common in female college students, female medical college students with AUB have multiple concurrent abnormal bleeding patterns. Factors associated with AUB varied according to the type of AUB, with sleep quality correlates with both types of AUB.

ABSTRACT

Objective:To predict the financial impact of artificial intelligence (AI)-assisted community-based screening for primary angle-closure glaucoma (PACG) among the elderly on health expenses and medical insurance expenditures in remote areas of China. Methods:A total of 19 395 people aged more than 65 years old from Changjiang Li autonomous county, Hainan province were enrolled.A health economic model for glaucoma screening in Wenzhou, Zhejiang province was adopted.PACG was classified as suspected primary angle closure, primary angle closure, PACG, monocular blindness and binocular blindness according to its severity. The model was adjusted in combination with the epidemiology, metastasis probability and treatment compliance, screening and referral compliance, sensitivity and specificity of AI screening from glaucoma related literature and using data on local epidemiology and health costs of glaucoma from the Changjiang Hospital of Integrated Traditional Chinese and Western Medicine from 2016 to 2019.The health outcome, total health expenses and medical insurance expenditures of AI-assisted community-based screening and no screening for PACG in 15 years were compared and analyzed. Results:It is found that the AI-assisted community-based screening would prevent disease progression and reduce the number of PACG patients by 43% (134 cases) and blind patients by 50% (9 cases) in 15 years in comparison with no screening.Under the AI-assisted community-based screening system, the early diagnosis and intervention of positive PACG patients will cause the dramatic increase of both total health expenses and medical insurance expenditures compared with no screening in the short term.It was predicted that the increasing trend of medical insurance expenditure would turn to a downward trend and show cost saving advantage at the third year, whereas the declining trend of total health expense would not occur until 10 years later.Without screening, a large number of PACG patients would progress to advanced stages, so the medical costs would continue to rise and exceed the medical costs of the screening system at the 14th year. Conclusions:The AI-assisted community-based screening for PACG is cost-effective and can prevent disease progression, save health expenses and medical insurance expenditures in the long run in remote areas of China.

ABSTRACT

Objective To investigate the effect of weakness on perioperative complications of thyroid, breast and hernia surgery in general surgery. Methods Patients receiving thyroid breast surgery in Liaoning Provincial People′s Hospital were randomly selected from May 2014 to May 2017. The demographic variables of each patient were recorded, and the demographic variables were stratified. Each group of people was scored with a weakness index (mFI), with complications and serious complications as the dependent variables, and the demographic variables and mFI scores were used as covariates. Cox proportional hazard regression analysis of covariates and dependent variables was performed, the covariates were stratified, and risk factors between subgroups were compared. Results A total of 1 408 patients were eligible for inclusion in the above inclusion criteria. The female population was 801, with an average age of (59.7 ± 11.6) years, an average body mass index (BMI) of (28.7 ± 7.4) kg/m2. Compared with those in low mFI, the risk of complications and serious complications increased significantly (P <0.01) in moderate mFI ( HR=1.60, HR=2.10), and high mFI ( HR=3.36, HR=3.96). Patients with≥80 years old ( HR=1.22, HR=1.33) had a significant increase in the risk of postoperative complications and serious complications ( P < 0.05). Long-term dialysis ( HR=2.20, HR=2.13), long-term application of glucocorticoids ( HR=1.51, HR=2.27) patients had significant risk of complications and serious complications ( P < 0.01). Local anesthesia monitor ( HR=0.63, HR=0.67) reduced the risk of postoperative complications and serious complications (P<0.05). The risk of complications and serious complicationsin patients receiving hernia repair with moderate mFI ( HR=1.88, HR=2.01) and high mFI ( HR=2.12, HR=2.80) was significantly higher than that in patients with lower mFI ( P<0.01). The risk of complications and serious complications in patients of breast cancer with moderate mFI ( HR=1.76, HR=2.43) and high mFI ( HR=1.97, HR=2.38) was significantly higher than that in patients of breast cancer with low mFI ( P<0.01). Patients of thyroid cancer with a high mFI ( HR=6.38) showed a significantly higher risk of postoperative complications, compared with those with a low mFI (P<0.01). Conclusions For patients undergoing surgery for hernia, breast cancer, and thyroid cancer, weakness is associated with perioperative complications.

ABSTRACT

AIM:To observe the expression of calreticulin(CRT)in nasopharyngeal carcinoma tissues,ana-lyze the significance of clinical pathology and the influence on epithelial -mesencymal transition(EMT)of CNE2 cells. METHODS:The expression of calreticulin was detected by immunohistochemistry in 52 nasopharyngeal carcinoma and 57 nasopharyngeal benign tissues, and the significance of clinical pathology was evaluated. The calreticulin gene-specific small interfering RNA was constructed, and then was transfected into the NPC cell line CNE 2 using the cationic liposome meth-od.The effect of CRT on the morphological changes of the CNE 2 cells was observed under light microscope. The effect of CRT on the cell migration and invasion abilities of the CNE 2 cells was detected by Transwell migration and invasion assays. The expression of EMT-related proteins E-cadherin,vimentin,transforming growth factor(TGF)-βand matrix metallopro-teinase(MMP)-9 in the CNE2 cells was determined by Western blot.RESULTS:The positive expression rate of CRT in the benign lesion tissues was 19.29%(11/57),which was significantly increased in the nasopharyngeal carcinoma tissues as 82.69%(43/52).The expression rate of CRT was positively correlated with the stage of nasopharyngeal carcinoma and lymph node metastasis(P<0.05).Knockdown of CRT expression made the CNE 2 cells showing a spindle shape to a flat, cobblestone-like epithelial state change, arranged more compact, and the migration and invasion abilities were significantly decreased(P<0.05).Knockdown of CRT expression resulted in significant increase in the protein expression of E -cadhe-rin,and the decreases in the protein expression of vimentin, TGF-βand MMP-9 in the CNE2 cells(P<0.05).CON-CLUSION:Calreticulin expression in nasopharyngeal carcinoma is significantly higher and positively correlated with naso -pharyngeal carcinoma stage and lymph node metastasis.Calreticulin promotes cell migration and invasion of nasopharyngeal carcinoma CNE2 cells by inducing EMT.

ABSTRACT

Objective To investigate the clinical effect of Zhuyun Ning Recipe Ⅱ in treating spleen-kidney deficiency type of threatened abortion with polycystic ovarian syndrome(PCOS). Methods A total of 40 qualified outpatients and inpatients admitted in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were included into the study. Twenty patients diagnosed as spleen-kidney deficiency type of threatened abortion with PCOS complicated with insulin resistance (IR) were included into group A, 20 patients with spleen-kidney deficiency type of PCOS non-IR threatened abortion were included into group B,and 20 normal women with early pregnancy were included into group C. Groups A and B were given oral use of Zhuyun Ning RecipeⅡfor 30 days, and group C did not receive any medication. The scores of traditional Chinese medicine (TCM) symptoms, blood beta-human chorionic gonadotropin (β-HCG), serum progesterone, homeostatic model assessment for insulin resistance index(HOMA-IR), interleukin-18(IL-18), and interleukin-20(IL-20) of groups A and B were observed before and after treatment, and were compared to those of group C. Results (1)After treatment for 30 days, the observation indexes of group A were much improved(P < 0.05 or P < 0.01 compared with those before treatment), and the indexes in group B except for HOMA-IR were also improved (P < 0.05 or P < 0.01). The inter-group comparison showed that the differences of blood β-HCG, serum progesterone, and IL-10 were insignificant (P > 0.05);HOMA-IR and IL-18 levels in group A were higher than those in groups B and C, and the differences were significant (P < 0.01). However, the differences of HOMA-IR and IL-18 were insignificant between group B and group C (P > 0.05).(2)During the treatment, neither adverse reaction nor toxic or side effect was found.(3)The follow-up results showed that 5 cases of group A,2 cases of group B,and 4 cases of group C had full-term normal delivery, and the health state of newborn babies was quite well. One case of group A had pregnant diabetes at late pregnant stage, and the other patients of group A got through the middle and late pregnancy, without any pregnant complications. Conclusion Zhuyun Ning Recipe Ⅱ is effective on relieving symptoms of threatened abortion patients with PCOS,and can improve the related laboratory indexes and pregnancy outcomes of the patients.

ABSTRACT

Background and purpose: Short tandem repeats (STR) multiplex PCR fluorescence detection technology is the most widely used DNA technology in individual identity and genetic identification. It's the most direct method to obtain accurate conclusions. However, some studies have indicated that the rate of STR mutations in tumor tissue is significantly higher than that in normal tissues or blood. This study aimed to investigate the tendency of genetic instability in 20 STR loci on autosomal and Amel loci in tumor tissue samples from lung cancer. Methods: This study, collected 75 cases of human lung cancer tissues and the adjacent normal tissues. DNA samples were extracted by tissue DNA extraction kit, amplified using MicroreaderTM 21 Direct ID System PCR amplification kit. Capillary electrophoresis was performed using API 3130 analyzer, and results were analyzed by genetic analysis software (Gene Mapper ID V3.2). Results: STR alterations were detected in 24 specimens from 75 lung cancer tissues (32%). Fifty-five alterations were detected in the frequently used 21 STR loci in total, including additional alleles 10 times, loss of heterozygosity 10 times, partial loss of heterozygosity 35 times. Partial loss of heterozygosity was the most common genetic alteration types accounting for 63.64% of the total alteration frequency. And multiple genetic alteration types could occur in the same lung cancer tissue. Among them, the highest alteration frequency occurred on D5S818 (7 times), secondly on D3S1358 and D12S391 (both 5 times), and no alterations on D2S441 and Penta E. Combining the experimental results and analysis on clinical data, this study found the statistical differences between the staging of lung cancer and the age of the patients with the STR loci alterations (P0.05). Conclusion: STR loci of the lung cancer tissue were not stable, and the alteration occurred in the aged or high malignant degree lung cancer tissue more frequently. Meanwhile, no alteration was detected on D2S441 and Penta E. In the future research the two STR loci should be verified to determine whether they can be used as the stable STR loci in such cases by increasing the sample size.

ABSTRACT

PURPOSETo determine the effect of the posterior condylar offset (PCO) on clinical results after total knee arthroplasty (TKA) using a high-flex posterior-stabilized (PS) fixed-bearing prosthesis.

METHODSWe prospectively studied the clinical and radiographic materials of 89 consecutive female patients (89 knees), who had undergone primary TKAs for end-stage osteoarthritis. All operations were performed by a single senior surgeon or under his supervision using the same operative technique. Based on the corrected PCO change, we divided all cases into two groups: group A (corrected PCO change ≥0 mm, 58 knees) and group B (corrected PCO change<0 mm, 31 knees). One-year postoperatively, clinical and radiographic variables from the two groups were compared by independent t-test. The associations between the corrected PCO changes and the improvements of clinical variables in all patients were analyzed by Pearson linear correlation.

RESULTSOne-year postoperatively, the Knee Society Scores, the Western Ontario and McMaster Universities Osteoarthritis Index, non-weight-bearing active and passive range of knee flexion, flexion contracture, extensor lag, and their improvements had no statistical differences between the two groups (all p>0.05). The corrected PCO change was not significantly correlated with the improvement of any clinical variable (all p>0.05). Group A demonstrated greater flexion than group B during active weight bearing (p<0.05).

CONCLUSIONSRestoration of PCO plays an important role in the optimization of active knee flexion during weight-bearing conditions after posterior-stabilized TKA, while it has no benefit to non-weight-bearing knee flexion or any other clinical result.

ABSTRACT

OBJECTIVETo explore the usefulness of head-space gas chromatography for the determination of methanol and ethanol in Haikesu I,a raw material of artificial musk.

METHODSThe determination was performed on HP-5(30 m×0.53 mm,5 Μm)capillary column with an hydrogen flame ionization detector. The solvent was dimethyl sulfoxide and the internal standard was acetone. The injector temperature and the detector temperature were controlled at 180 ℃ and 250 ℃,respectively. The carrier gas was nitrogen. The containers of head-space injector were preheated at 90 ℃ for 15 minutes. The column temperature was programmed raised, which achieved baseline separation of the components.

RESULTSThe results showed a good linear relationship for methanol and ethanol in their linearity range;and the limit of detection was 0.8 and 1.0 Μg/ml,respectively. The precision and average recoveries were satisfactory.

CONCLUSIONThe head-space gas chromatography is simple, rapid, and precise technique for the measurement of residual solvents in Haikesu I.

ABSTRACT

Objective: To explore influence of coping style on sleep quality and blood pressure in community male population with high normal value blood pressure. Methods: The Pittsburgh sleep quality index (PSQI) and coping style questionnaire (CSQ) were used to assess 120 men with high normal blood pressure in community. With PSQI>7 scores as criterion for judging sleep quality disorders, the subjects were divided into sleep disorder group (n=51) and normal sleep group (n=69), and sleep disorder group received psychological intervention. Results: Sleep disorders existed in 42.5% male population with high normal blood pressure. Compared with normal sleep group, there was significant increase in PSQI [(6.43±2.59)scores vs. (8.33±3.14)scores] and diastolic blood pressure [(81.00±8.91) mmHg vs. (88.00±5.69) mmHg] and significant decrease in factor scores of “problem solving” [(0.76±0.21) scores vs. (0.61±0.18) scores] and “asking for help” [(0.52±0.26) scores vs. (0.41±0.11) scores] in sleep disorder group, P<0.05 all; Compared with before intervention there were significant increase in scores of “problem solving” [(0.61±0.18) scores vs. (0.71±0.12) scores]and “asking for help” [(0.41±0.11) scores vs. ( 0.51±0.13) scores]and significant decrease in PSQI score [(8.33±3.14) scores vs. (7.41±2.37) scores] and diastolic blood pressure [(88±5.69)mmHg vs. (80± 4.17)mmHg] after psychological intervention 12 weeks in sleep disorder group, P<0.05 all. Conclusion: Psychological intervention may improve sleep quality and reduce blood pressure in community male population with high normal value blood pressure.
